Please consider this my registration for the above-mentioned event organized by Spring Gallery Artist Group and Cabarrus Art Guild (both of Concord, North Carolina), and to use the property and facilities of the First Presbyterian Church located at the Memorial Garden at 36 Spring St., Concord, NC 28025.

I agree to observe and obey all posted rules and warnings and further agree to follow any oral instructions or directions given by the event organizers.

I release and discharge the event organizers and property holders mentioned above for any injury loss or damages that may result at this event or during the use of any facilities at this event.

I ask that the event organizers to treat this as my signature. I agree to follow all posted rules.
